Our Adult Day Centers provide comprehensive and compassionate care, designed to enhance the well-being of our patients. We offer skilled nursing services directly provided by the order of the patient's attending physician, health monitoring, and personalized care including grooming and assistance with daily living. Patients enjoy nutritious home-cooked meals and snacks, engaging individual and group activities, physical fitness activities, and community involvement including fun monthly outings. Our centers promote social interaction which plays a vital role in both mental and emotional well-being of our patients. This helps to combat feelings of loneliness and isolation through connection and community that can significantly enhance their quality of life. We maintain the flexibility to meet the unique needs of each individual.
At our centers, we are proud participants in the Child and Adult Care Food Program (CACFP).
This vital federal program allows us to provide healthy, balanced meals and snacks to all patients in our care. Our participation ensures that the food we serve meets strict U.S. Department of Agriculture (USDA) nutrition standards, contributing to the wellness, healthy growth, and development of every participant.
By offering high-quality, nutritious food throughout the day, we help participants establish positive eating habits and are committed to serving a variety of wholesome foods, including more whole grains, fruits, and vegetables.

Our experienced Case Managers work diligently to assess individual situations, develop personalized care plans, and coordinate access to essential services. We act as advocates, connecting clients with healthcare providers, community resources, and support networks. We guide participants through the Medicaid process, ensuring they receive the support needed to enhance their quality of life. By leveraging our expertise and understanding of Kentucky Medicaid regulations, our goal is to streamline the care process, reduce burdens, and empower individuals to achieve their optimal well-being by ensuring they receive the appropriate and timely assistance they require.
Our Case Management services extend beyond coordination of care, encompassing assistance in securing essential goods and services. Our experienced Case Managers work diligently to identify and facilitate access to necessary medical supplies, durable medical equipment, home adaptations, and other eligible resources.
We understand that healthy eating is a cornerstone of overall well-being. That's why we've partnered with trusted third-party providers to facilitate home-delivered meals. Through our case management services, we help ensure participants receive nutritious, convenient meals tailored to their needs. This essential support empowers individuals to maintain independence and live a healthier life in their own home and community.
Our Adult Health Centers offer valuable respite care, providing temporary relief for family caregivers while ensuring their loved ones receive attentive and engaging support. Our respite care services offer a safe and stimulating environment where patients can enjoy social interaction, participate in enriching activities, and receive personalized care. This allows caregivers to take a much-needed break, knowing their loved ones are in capable and compassionate hands. Waiver Services provide comprehensive support through Case Management and Adult Health Care to individuals with intellectual or developmental disabilities, empowering them to live fulfilling lives within their communities. Our experienced Case Managers act as advocates, working closely with individuals and their families to develop personalized care plans that address unique needs and goals. We facilitate access to essential services, including personal care, respite care, therapy, and community resources, ensuring individuals receive the support necessary to thrive. By navigating the complexities of the Michelle P. Waiver program, we strive to enhance independence, promote self-determination, and improve the overall quality of life for those we serve.
